On Your First Visit
Medical Referral Letter
Since this is an advanced treatment hospital, we ask that you bring a medical referral letter from your home doctor on your first visit. Otherwise, you will be charged a 5,500 yen out-of-pocket special service fee.
Appointments
You may ask your home doctor to make an appointment with us, or you may make an appointment on your own (in Japanese only). The direct line to the appointment desk is 0263-37-3500.
NOTE: Unless you have made an appointment for your initial consultation in advance, you may be refused treatment in some departments.
What to bring（if you have them）
- Your health insurance card
- A medical referral letter
